Procedure:

1. You will need Adobe Acrobat Reader to view the PDF files. Click HERE or the icon below for a FREE download of Adobe Acrobat Reader. Please follow the installation instructions to install Adobe Acrobat Reader in your computer.
    
2. Click on the links above to open the selected file in a separate browser window.
    
3. To print the file, select "Print" option.
    
4. In the case of the Medical Certificate, please have your child's pediatrician accomplish and sign the printed form.
    
5. Submit the accomplished forms to the FMPLI office. School hours are from 8:00 a.m. to 4:00 p.m., Monday to Friday. Saturdays by appointment only.
